Performax Hockey player ID and Registration Process (Rev 1/06)
The first (2) weeks of each season a table will be set up in the lobby area of the Amherst Pepsi Center in order for you to fill out the USA Hockey insurance & registration form. We will have the table set up from 1 hour prior to the first game of the night until the last game starts that night. After the first (2) weeks of the season arrangements must be made through your captain or by calling Ed Ellis at 418-2098.
All participants must fill out the form and pay for the insurance before taking the ice for any Performax Hockey event. The form will cover both the registration for USA Hockey and Performax Hockey. A digital picture will also be taken for ID purposes. There is also an additional waiver form to sign.
USA Hockey insurance cost for the current season is $30. Performax and USA Hockey registration and ID cost is an additional $10. Total cost is $40 and can be paid via cash or personal/business check made out to Performax Hockey. This fee is paid at the table and is separate from the fee paid to the captain for the team fee.
If you have paid for and have proof of having obtained USA Hockey insurance from another adult league, organization, or directly from USA Hockey’s internet site or are a youth hockey coach you will only need to pay the $10 registration and ID fee.
Any questions or concerns about what USA Hockey insurance covers can be answered at their usahockey.com web site. USA Hockey insurance is not meant to replace your personal medical insurance. The "excess medical" coverage is designed to supplement your primary (any other insurance) coverage that you have, and it only pays for treatments AFTER the primary has been used.
